A Deed Poll (Change of Name – Married Woman) is a legally binding document used by a married woman to adopt her spouse’s surname formally, revert to her maiden name, add or remove names, or align her official records following marriage.

This is an application for change of name by a married woman. It contains:

• A Compliant Deed Poll;
• Consent by the Husband or if divorced a Certificate of an Advocate; and
• A Statutory Declaration by a witness who has known the applicant for at least ten (10) years.

The document enables a married woman to legally renounce her former name and adopt a new name for all personal, professional, and official purposes. It is suitable for registration under the Registration of Documents Act (Cap 285) and may be used for publication in the Kenya Gazette where required.
